Patients scheduled for appointments will receive a text confirmation, if a textable number is provided. Once confirmed, you will receive a link to be able to fill out your paperwork. if you are unable to do so, please arrive 15 minutes prior to your appointment time to fill out paperwork in our office.
Patients are required to fill out this paperwork once a year.
